A program in which contracts are established with providers of medical care; providers under such contracts referred to as preferred providers; usually provides significantly better benefits (lower copayments) for services received from preferred providers, thus encouraging covered persons to use these providers; covered persons generally allowed benefits for nonparticipating providers’ services, usually on an indemnity basis, with significantly higher copayments.
